Foreign tax experts please help.

Client has a life insurance policy in Hong Kong and he is still making premiums. I heard of an 1% excise tax to be charged on the premiums. Two questions:

(1) Form 720 which is used to pay the excise tax has a space for Employer ID number only. Obviously my client is not in business and he does not have an EIN. So shall he apply for an EIN just to file the Form 720? Or can he put in his SSN?

(2) I hear there are certain countries that the excise tax on life insurance policy are waived based on tax treaty. The life insurance policy of this client is in Hong Kong. So does US and Hong Kong has a tax treaty that waiver the excise tax on it?
