Although I understand it is perhaps a best practice to have the parent indicate signing capacity when signing a Form W-9 on behalf of the parent's minor child (e.g., "/s/ Billy Doe by Mary Doe, Parent" or "/s/ Billy Doe by Mary Doe, Guardian"), if Mary Doe is in fact Billy Doe's parent, is a Form W-9 signed "/s/ Billy Doe by Mary Doe" invalid? I received a Form W-9 signed by parent without capacity and am trying to decide whether need to request that a new Form W-9 be signed by parent indicating signing capacity. Seems silly to require parent to sign a new Form W-9 but wanted to forum feedback on issue.

If you know the signer is the parent of the minor, I think it's fine.
